Telephysio

I am offering free telephysio or online video consultations which are secure to all parents of children residing in Ireland up until May 16th 2020. I understand that there can be long waiting times for therapies for certain children hence the offer of free video consultations. I have started a clinical practice in Co. Monaghan but with the current restrictions in place I have decided to continue on with video consultations to meet demand. I have found the video consultations to be very effective from the point of view that the child is observed in their own home where they are most comfortable and perform like they would normally which improves the quality and accuracy of assessment and follow on treatment. The video assessments are observational and play based for the younger children and more functional task based for the older children. Videos are never recorded and are easy to set up as long as the parent has a laptop or ipad or smart phone with an integrated camera. The approach I take is the goal directed task oriented approach which is in line with best practice in paediatric physiotherapy and is suitable for video consultations.

After May 2020, fees for telephysio/online video consultations are as follows:

Initial assessment (1 hour): 50 euro

Follow up treatment (30 mins): 30 euro

Initial Assessment

During the Covid 19 pandemic up until May 16th 2020, I will offer a free online video consultation to determine your child's physiotherapy requirements. All you need is a smart phone or ipad or laptop with a webcam for the video consultation. I will email you a link to access the video consultation. For the consultation I will ask you to turn your camera to video your child so I can observe her/him playing as she/he normally would in her/his home environment. For older children, I will ask them to perform certain functional tasks depending on their ability levels. I will also be assessing movement patterns, abilities, functional range of movement and strength which can be assessed via video. Video consultations will never be recorded. Only tasks will be carried out as part of the video assessment procedure with parental consent. Based on the assessment findings and in consultation and agreement with the child and parents, a physiotherapy programme will be devised and the child (as age allows) and parents will be coached how to implement this at home, in school, everyday life as appropriate. Goals will also be set and agreed. Follow up if desired and as appropriate will be agreed and scheduled. A few distinct advantages to video consultations is convenience for parents and children, children are generally more comfortable in their own homes and therefore perform more naturally which enhances the accuracy of assessment. Video consultations are less costly such as less or no time off work/school, cheaper than clinic based appointments and no travel costs. They have also been shown to be as effective as clinic based appointments in research to date. Please refer to telephysio services below for fees after May 2020.

During the Covid 19 pandemic, follow up appointments can take place by video consultation. Individualised goals focused play and routines based therapy programmes will be monitored, reviewed and progressed as appropriate. Please refer to telephysio services below for follow up fees.

Cross Border Healthcare Directive

The Cross Border Healthcare Directive is operated by the Health Service Executive (HSE). It allows individuals on a HSE waiting list or if in a service already via a GP referral to access Physiotherapy treatment (and other treatments) within Northern Ireland or other EU member states; thus receiving treatment ASAP.

The HSE can reimburse costs incurred; repayment is usually the amount that the treatment would cost in Ireland or the cost of your treatment in Northern Ireland, if that is less.

This means that if your child is on a waiting list for Physiotherapy or is in a Physiotherapy service already but your child requires extra physiotherapy you could attend the Cookstown Move Kids Physio clinic in Co. Tyrone, receive specialist paediatric physiotherapy from a highly experienced and skilled paediatric Chartered Physiotherapist and apply to have costs

reimbursed. Please refer to the Cross Border Healthcare Directive page on the HSE website for more information:

Initial Assessment

The initial assessment appointment can take place in clinic (Cookstown Co. Tyrone) or by video consultation.

There are two types of paediatric assessment:

1. Paediatric Neurodevelopmental Assessment which is 60 minutes and includes a thorough examination of your child's muscle tone, muscle strength, flexibility, biomechanics, posture, movement patterns & standardised assessment of balance, co-ordination & motor skills following a short interview with parents to get some background information. The initial assessment includes a home programme as needed.

2. Paediatric Musculoskeletal Assessment which is 45 minutes long and includes a thorough examination of your child's muscle tone, muscle strength, flexibility, biomechanics, posture & movement patterns following a short interview with parents to get some backgorund information. This assessment does not include standardised assessment of balance, co-ordination and motor skills.

The initial assessment includes a home programme as needed.

Both assessments include follow up email with home programme as needed. Letters and reports are charged extra to assessments.

Follow up Sessions

Follow up sessions can be agreed and scheduled at the initial consultation. Follow up appointments can take place in clinic or via remote video consultation depending on client preference. Follow up appointments are charged at a lower cost than the assessments due to the extra time and administration involved with initial paediatric assessment.

Home visits

Depending on your child and family circumstances and on Covid 19 restrictions, I am available for home visits, depending on location and if appropriate. Fees will apply depending on travel and location. Please contact me for further details.

Video consultations

Telephysiotherapy is the development of telemonitoring systems such as online video consultations to facilitate independent rehabilitations of individuals within their own homes. Telephysiotherapy is a convenient way for children and their families to avoid travelling and can be more real life focused than clinic based appointments as children are assessed and treated within their own homes. There is a move in children's rehabilitation away from traditional 'hands on' therapy toward more effective evidence based and family centred approaches including child and parent coaching and goal directed, task oriented and routines based interventions. These new approaches are highly compatible with online video consultations.

A few distinct advantages to video consultations are:

  • Convenience for parents and children

  • Improved flexibility - appointments can be arranged that suits the child's and family's routine/schedule.

  • Less cost - less time off work/school, cheaper than clinic based appointments, no travel costs and environmentally friendly.

  • As effective as clinic based appointments - remote video consultations have also been shown to be as effective as clinic based appointments in research to date.

The video assessments are observational and play based depending on the child's age. The online video platform that I use is GDPR compliant and therefore secure. Videos are never recorded and are easy to set up as long as the parent or carer has a laptop or smart phone with a webcam. The approach I take is the goal directed task oriented play based approach which is in line with best practice in paediatric physiotherapy and is suitable for video consultations.
